He suffers from delirium along with obstruction of speech, which does not get relief by any means. Maha hikka is mahamula (deep rooted), mahavega (massive attack), mahashabda (very loud sound), maha bala (very strong). This can lead to death immediately, hence named as mahahikka. (21-26)
